Ensuring Successful Delivery from the Start
Successful programme and Project delivery depends on having the right knowledge, experience, and delivery models in place when you need them.
The greater the complexity of the change, and/or the lower the experience of the organisation and its suppliers in delivering that change, the greater the risk that the delivery will run into serious issues. If a programme or project is not set up correctly, then realisation of these risks becomes a certainty.

Alignement is the Most Fundamental of the Factors
Whilst localised issues with Objectives, Resources, Planning and Expertise will cause a Programme serious issues, a lack of alignment will likely cause issues in some or all other areas, and is most likely to lead to overall failure.
Realisation & Recovery
It can often take some time for enough indicators to emerge for senior stakeholders to realise a Project is in serious trouble. This "fog of war" is frequently made worse by deficient reporting.
If the Project is still viable, then you are in recovery plan territory, and that always comes at a high cost in terms of money, delays, and the reputations of individuals, departments, and the organisation itself, sometimes very publicly.
At this point external expertise is often brought in and the Programme or Project is put back on track, but this time with sufficient delivery assurance and safeguards in place.
With all Success Factors Present
"Sufficient" is an important word. This is not about deploying an army of checkers or reams of more reporting, it is about having the right measures in place, scaled appropriately to the size and value of your delivery.
Throughout this process the confidence of the stakeholders goes through a roller-coaster ride as shown in the diagram below, from a "false comfort" to "a real comfort", based on a well run delivery built on strong Programme & Project Assurance foundations.

Appling sufficiently robust Delivery Assurance measures from the start, and throughout the lifecycle, will pay dividends in the cost and quality of any delivery.
This approach avoids the need for a "Retrospective" path to delivery Assurance and will always enhance the reputation of the organisation with both your internal and external customers.
